About our service

We provide flexible NDIS-funded housing options to support a safe and timely transition from hospital to home. This includes Supported Independent Living (SIL) for ongoing daily support, Medium Term Accommodation (MTA) for short-term transitional housing, and Specialist Disability Accommodation (SDA) for participants with high physical support or specialist housing needs.

We work closely with hospital discharge teams to ensure the right accommodation and supports are in place, helping participants move confidently into a suitable home environment that supports recovery, stability and long-term independence.
